During the production of "Citizen Kane"...

I have two questions:

1...At what point did the title of the movie get changed from "American" to "Citizen Kane"?

2...While it was being filmed, did people in Hollywood (not connected with the movie) know that it was based on the life of Hearst? Or was it not until the movie was made and previewed that it dawned on people (like Louella Parsons) what Welles had made?
